Please contact your pharmacy to refill your medication, otherwise please use your my-chart account to request the refill directly from your provider.
Please call our office at 203-281-2890 ext 28 for additional assistance. If you are overdue for a follow up, you will be asked to kindly schedule a visit at the time of your refill request.

Our office requires a pre-screening intake prior to booking any new patient appointment. This intake is done over the phone with our intake coordinator to determine whether any of our providers are a good fit for you. The intake should not take no longer than 15 – 20 minutes.
Once you have been assigned a provider, please send us the following information:
- Form of Identification (i.e. driver’s license, school ID, or other)
- Insurance card – Front and Back
You will need to have a my-chart account set up as future Telehealth visits
will be going through your MyChart account.
